Permission Form

If your child is in grades 3-5 and he or she is interested in participating in cross country, please read the message below.
1. The cross-country meets start next Wednesday, September 21st. 
 
2. We have sent a permission form home to be signed. Can you please have this form signed by
   Monday, September 19th? In case your child has lost his or her form, I have attached it as a 
   document of this page.
 
3. We will have a bus to transport all children to the meets; however, you are responsible to
    pick up your child at 2:15pm at the location of the meet. 
 
4. The cost of the bus for the 4 weeks is $4.00. Please send that in when you send in your
    child's permission form.
 
5. Students will be eating lunch at school before the meets; so, please pack a lunch and lots of
    water for the next  4 Wednesdays.
